Homework
1. Required: Oral Health Requirement
OHA requires all CHWs to take an Oral Health Training. Your Oral Health Training certificate is required to receive your CHW training certificate. Please share your certificate with bethm@15thnight.org and keep a copy for your records.
2. Utilizing a Community Health Assessment or 5/20/26 Mapleton School District Field Trip.
This homework is required if you are unable to attend the 5/20 Mapleton School District Field Trip.
3. Health Literacy Homework or 6/5/26 Eugene Field Trip.
This homework is required if you are unable to attend the 6/5 Eugene Field Trip.
